I hereby authorize the lifeguard staff of NVPools, Inc. to obtain emergency medical care for injuries or illness for my child that might occur while at the pool facility (“the Club”) and events attended by the swim team, which exists as part of the Club. I direct all medical facilities to accept this document as authorization to render essential medical care necessary in the event of an emergency and I am unable to be immediately contacted.
Swim Club Rules
I agree to abide by the rules of the Forest Hollow Swim Club as outlined at www.ForestHollow.com.
Photo Release
Registration constitutes agreement by the registrant and family members to Forest Hollow’s and the swim team’s use and distribution of registrant’s image for use in promotion.
Emergency Release
I give permission for my household members to participate with the Club and Swim Team (which exists as part of the Club). I understand that swimming may be a potentially dangerous sport, with the possibility of injury, and I acknowledge that it is my responsibility to provide insurance coverage and to pay for treatment to be administered to my members in the event of any accident, injury or illness, until such time as I can be contacted. I assume all responsibility for payment of any such medical treatment that may be necessary.
I give my permission and approval for participation of registered adults and children under my membership, in any and all activities sponsored by the Club and I assume all risk and hazard incident to such participation, including transportation to and from such activities. I waive, release, indemnify and agree to hold harmless the Club, Assistant Coaches, Employees, Club Officers, officials, participants and parents from any claim arising out of injury to my child/children while participation in any and all activities, including, but not limited to transportation to and from all practice and competitive meets sponsored by the Club.
I know of no impairment or deficiency, physical health or otherwise, that would limit or prohibit my child/children from participating in practice sessions and competing with other children. I agree to advise and make known to the Club Officials and Coaches any change in the physical health or any other condition that would limit or prohibit my child/children from participating in practice sessions and competing with other children.

I have read and accept the responsibilities and standards of conduct outlined in the NVSL Standards of Conduct, posted on the NVSL website.
